Claim Form(s) 1. The Form(s) for claims submitted under this program must be submitted to the State Controller's Office. There is no charge for claims filed in this manner. You may download the form(s) from this page. 2. A Claim Form/Request for Reimbursement will be e-mailed to the claimant's e-mail address on file with SCO. If your e-mail address becomes inactive or has been deleted, contact the SCO. 3. If claimants pay the claim, the SCO will reimburse the claimant's payment with an Electronic Funds Transfer (EFT) PIN. The claimant will also receive a copy of the reimbursement form. This form must be submitted with the Claim Form(s). The claimant must have an active EFT PIN on file with SCO to submit the Claim Form(s). In addition, the claimant is responsible for paying to reimburse SCO all expenses relating to SCO processing of the claimants claims. SCO will not issue a claim or release a bill of any kind without a valid EFT PIN. All claims and attachments must be filed online and in English, with proper formatting. (Claim Form(s) and additional information regarding the claim form(s) are provided on this link: ) 4. If a claimant agrees to enter into arbitration, or the claimant does not receive a valid response to the letter sent by SCO to request information on the amount of expenses for which reimbursement is requested, the claimant must submit an affidavit with the claim that includes all the information requested by the SCO. A claimant who files a claim in accordance with the instructions on this link will receive a request for more information within 30 days of SCO initiating the claim or any time thereafter. The claimant will have an opportunity to resubmit the affidavit with all the required information. 5. The State Controller's Office will retain funds for the claimant to reimburse the claimant for expenses incurred as a result of the State mandated cost program. The claimant must file a claim within 45 days and file the claim with all correct documentation within 90 days. If SCO rejects a claimant s claim for reimbursement, the claimant will remain responsible for paying the SCO. SCO will make an assessment of the claimant s expenses within three (3) years of the date of rejection.
