This limit and the other benefits described in this policy are subject to change as additional regulatory guidance becomes available; prior year's deductible or out-of-pocket maximums in effect prior to the effective date of changes to these policies will be maintained. This plan is intended to comply with the federal Patient Protection and Affordable Care Act. Provisions are subject to change as additional regulatory guidance becomes available. This plan is designed to increase the number of uninsured and low-income families who participate in the health plan market and who have a reasonable cost of health insurance for health care related expenses. In order to reduce the number of families who are uninsured and uninsured for too long, we may increase the annual deductible and/or deductible percentage, deductibles and/or maximum amounts for certain health care services and health care related expenses. Please note, this policy does not limit the ability of a family to purchase additional benefits at any time. The maximum benefit under this policy may decrease if other benefits under this plan, such as health savings accounts, employer contributions to health benefits, prescription drug coverage, dental coverage and other benefits were offered during the year. The maximum premium and deductible percentages in effect prior to the effective date of changes to the maximum benefits under this plan will be maintained. This plan is designed to improve the market participation of the plan participants and other persons or entities that are reasonably likely to be eligible for the plan's benefits. This plan provides coverage against all forms of medical debt, including but not limited to unpaid health care debts, and does not limit the ability of the plan participant, group health plan sponsor, beneficiary or health plans offered under any health care contract to increase or decrease benefits in subsequent years. The annual limit on benefits is one-half of the total amount of benefits for coverage periods preceding the coverage period in which the annual limit was exceeded. The maximum benefits that will be paid for any one event under this plan, regardless of the number of occurrences of that event, is 0. You are automatically enrolled in the plan.
