The payment schedule for each procedure or treatment is designed to enable Medicare to recover sufficient amounts to cover all the actual Medicare charges. Physicians who bill using a prescription ordering system do not have to make a payment to Medicare on each medicine. Only those payments that Medicare is entitled to recover under existing program regulations that have been signed by the practitioner and are retained by the practitioner for administrative purposes may be made to Medicare. Medicare charges for each item billed to Medicare include the practice expense, the amounts Medicare has to reimburse the practitioner for, and the amount paid to the practitioner. In an effort to assist Medicare with the reconciliation of Medicare's claims and the practitioner reimbursement amounts, the contractor must include Medicare's claims on all the billing materials that it sends to Medicare. For certain payment instruments, such as the ICD-9-CM or ICD-10-CM, the supplier must include the following information for each item billed to Medicare: If the prescribing physician has received any Medicare provider reimbursement, then (A) the practitioner's name and address at the time the payment was made, and the date the payment was issued in accordance with procedures established by agreement between the physician and the supplier; (B) the name of each reimbursement provider (such as physician or hospital); and (C) the amount of each reimbursed item (the amount of Medicare payment and any Medicare cost-sharing charge that the provider is required to make to Medicare, as applicable) and the date of the reimbursement. The CMS has provided, at its expense, a computer program that is used by the contractor to record (1) the practitioner's prescription information with respect to each drug covered under Part D and each part of the DME in which pharmaceutical benefits are provided, (2) the physician's and practitioner reimbursement amount information with respect to each drug covered under Part D and each part of the DME in which pharmaceutical benefits are provided, and (3) the practitioner's practice expense information on each drug covered under Part D and each part of the DME. The program includes procedures as described in the CMS documentation for making these documents available to Medicare Contractors who use it to perform the obligations established by Medicare and by Medicare Contractors on behalf of Medicare.
