Application Process: Applicants should visit the DOLL office no later than the first business day of the month following their application. Applications not received within these time frame will not be processed. The DOLL office will provide applicants a “no-cost” receipt of receipt for all applications in progress. All applications are to be submitted electronically. There is a fee of 125.00 for online applications. A one-page document summary is included with each application. The summary should be in PDF format. A completed application form and all supporting documentation must be faxed or mailed to the DOLL office. DOLL will notify applicants of receipt of completed documents and provide a payment receipt. Application Package: To save time during processing, DOLL requires applicants to retain all supporting documents. A package will be created for each individual application and a single application will be retained by DOLL for processing on a rolling basis. The package and supporting documents will be mailed to all applicants in a mailing envelope. Mailing the completed package to the DOLL address is the best method of handling all documents. All applicants are responsible for shipping their application to DOLL no later than the first business day of the month following their receipt. In the event an application is delayed due to a failure to file a supporting paperwork, the applicant may reapply for approval at the DOLL office. The application fee is nonrefundable. The DOLL accepts personal checks, money orders, bank transfers, and personal checks and money orders with a financial institution. However, payment by credit card must be received at the DOLL office within 180 days of the date of the application. Any application that is not processed within this time period will result in the denial of licensure. Upon written request by a licensee prior to processing an application for licensing or continuing licensure, DOLL will provide a “no-cost receipt of receipt” that must include the following: The applicant's name. Date of birth Date application received Title of business Nature of business Licensing fee License application number (if any) Applicant's signature A copy of the license application/license and current license is available for inspection upon request to all licensees in DOLL offices. It is important that the applicant, in addition to filing an application to establish a business, provide all documents required to establish a business upon receipt of the completed application.
