Expectant Fathers, Birth Mothers, Infant Birth, and Infant Death Benefit This is a formal request for Aetna to cover medical emergency care at the preferred level of benefit and at the time of your desired birth. The request may be made in writing, at your convenience, and shall be made within a timeframe established by the Aetna Care Transition Coordinator, Health Services Unit – Transitional Care, Aetna, 1 Post Office Box, Hartford, Ct. 06.
Child Death Benefits Child death benefits are available if you die prior to or within 42 days after your first birthday, depending on whether you die in hospital or at home. Your spouse, parent or other adult member of your family may submit the child death benefit request for benefits at the same time you submit the child's plan request. If the other adult has been a dependent on the plan, they should receive a notice of approval for death benefits with the plan request. The notice includes further information about your child death benefit and the period for which your child's claim for disability benefits will be approved. If you die after you submit the child death benefit request, your benefits will be automatically extended for the length of time the beneficiary was on your individual or joint plan with your spouse, parent, or other adult member of your family during the period when you were receiving coverage, and may be in accordance with the current Aetna policy. If you die before you submit the child death benefit request, you should submit a new or separate request. (See also: Aetna death benefit question and answer) If all information on the original request is correct, we will respond as indicated on the request, and send you a copy of the notice of approval for child death benefits. If you are not able to locate the original child death benefit request or notice, or if you do not have the proper information, please complete this form (the completed form must be accompanied by a copy of your death certificate). You will receive a coverage decision by mail. The decision may take up to 30 days to be processed. If you need more time, you may send the request via fax or email to Aetna, 1 Post Office Box, Hartford, Ct. 06.
