(iii) upon issuance of a new federal registration number pursuant to subsection (g) of Section 5-313 of this title. (5) The provisions of this section relating to electronic transmission of the Form MTV 5-27 required by this section shall not apply. (6) The form shall include all the following, not inconsistent with this section: (a) The address, toll-free telephone number, and title registration form number of the Department. (b) A description of the method of payment of the fee required to be furnished by this section. (c) A statement of the applicable fees. (d) Instructions for the form. (e) Such terms and conditions as may be prescribed by the Department. (f) The form shall contain a “use for this purpose” statement. A form furnished under this subsection (g) shall contain a statement that the form can be used for establishing legal title to an Alabama certificate of title or to allow a purchaser of an Alabama certificate of title to establish ownership of an automobile for the personal, family, or household use, of the purchaser. The statement on the form shall be in substantially the following form: USE FOR THIS PURPOSE, THE FORM “MTV 5-27” HAS BEEN GIVEN TO YOU BY THE DEPARTMENT OF REVENUE.
