The current GSA must contain the following information: (A) The name of the sponsoring or subrecipient, (B) A description of the proposed activity, (C) The anticipated start date and completion date of the proposed activity, (D) Any additional information required for review, approval or monitoring of the proposed activity, and (E) The deadline for receipt of a completed GSA by the Arizona Department of Education. 3. The current GSA is a public document that may be obtained by anyone for review purposes, subject only to the disclosure provisions of § 16-8-102, et seq. 4. It must be signed by the sponsoring or subrecipient, by a representative of the sponsoring or subrecipient requesting review, and by an employee of the sponsoring or subrecipient. 5. This GSA is available to ALL State, school district, charter school, school district and charter school official(s) and their employees. 6. Information in the current GSA does not constitute a legal or professional privilege. 7. The provisions of this GSA can only be waived by the sponsor under the terms and conditions described in this GSA. 8. To obtain the GSA for your school district, your charter school, your state agency or institution of higher learning, or your community based organization (which is a governmental entity), you must: (A) Submit written request to the Arizona State Department of Education (ASIDE) and request waiver of non-public information, (B) Have your local district or charter school office or a representative review the GSA, and (C) Submit an authorized copy of your GSA to the ASIDE. Please note: This request must be received in writing and submitted no later than March 16, 2012, by the deadline specified in paragraph (A) above. 9.
