Great software, some things are not intuitive You can edit documents, replace text, add fillable fields, etc. Great for documents that are used often and just need a few details changed. "Replace text" can be difficult to use. Also the process of saving a document is kind of clunky.

There are five items that need to be addressed in the report: The scope of the FMEA. Who was involved with the FMEA and the time frame it was done in. Listing of the highest risks identified along with supporting information. Recommended (or completed) action plans. Suggestions for leveraging the FMEA study.
Completing the FMEA: List the INPUTS on left side of table from the Correlation Matrix. List each failure mode (or input that is or could contribute to the problem). Assign 1-10 for SEVERITY. Assign 1-10 for the frequency of OCCURRENCE. Assign 1-10 for the capability of DETECTION of the failure mode.
In the FMEA in Figure 1, for example, a flat tire severely affects the customer driving the car (rating of 10), but has a low level of occurrence (2) and can be detected fairly easily (3). Therefore, the RPN for this failure mode is 10 x 2 x 3 = 60.
Failure Modes and Effects Analysis (FMEA) is a systematic, proactive method for evaluating a process to identify where and how it might fail and to assess the relative impact of different failures, in order to identify the parts of the process that are most in need of change.
