How do I create a fillable PDF form with a fill button?
Open the PDF file that you will use for the form.
From the Forms menu, select Add or Edit Fields
From the Add New Field pull-down menu, select Show Tools on Toolbar.
Click Button Tool.
Position the cursor where you want the button to begin.

Choose Tools > Prepare Form, then select Button in the toolbar, and create a button.
Double-click the button to open the Button Properties dialog box.
