Who needs a DA 5434 Form?
The US Department of the Army Form 5434, Sponsorship Program Counseling and Information Sheet, ?is to be completed by military members who are about to make a Permanent Change of Station (PCS) move. The military are required to ask their new duty station for a sponsor. Upon the filer arrival, their sponsor shall act as their representative, provide them with necessary information and help to settle in a new place. It is mandatory that a sponsor should have an equal or higher rank than the arriving military.
