Had a chat with Paul on the site, and followed his suggestions for a fix.
Then was able to print the form. I am a new user and have yet to make changes to the form and print it.

Excellent!!! If this doesn't help you in today's busy office nothing will. I compare it to driving downtown. You know where you want to go but can't because of all the 1-way streets. It opens up all the roads.

That messed up a discount I had, but kudos to the customer service for being able to straighten it out. *** to do everything through chat because no phone number. Overall decent **************."
