How do I save a Google form response as a PDF?
Open your Google Form and click on the Results tab.
Click the 3 dots (more) and choose Print all responses.
In the Print Preview page, choose Open PDF in Preview (bottom left link).
All responses will open as a PDF in the Preview app.

Go to the tool menu and click the edit and add the text field and click the properties and click the action and select execute a menu item and select the Save as Button. Click ok.
What is a radio button in PDF?
Radio Buttons are used to allow users to select either a single option from a list of options in a PDF form. This is because radio buttons are used when only one option from the list should be selected. If another option in the same radio button group is selected then the other option will be unselected.
How do I save answers on a PDF?
To save the completed form, choose File > Save As and rename the file.
To remove extended Reader features, choose File > Save A Copy.
To allow Reader users to save the data they typed, choose File > Save As Other > Reader Extended PDF > Enable More Tools (Includes Form Fill-in & Save).
