I have found forms through PDFfiller that I could not locate anywhere else--wonderful tool! It's fairly intuitive, but there are some cumbersome issues related to printing--too many steps--could be simplified.

I am new to this program the search engine next to the description of the for would be great if you could see a picture of the for rather than having to click on it if it is not the one you want then back to the search engine

For pdfFiller’s FAQs

Below is a list of the most common customer questions. If you can’t find an answer to your question, please don’t hesitate to reach out to us.
Decide where to sign. Personalize your message. Choose a signature phrase. Make sure your name is legible. Add a date (optional). Use a good pen.
Use your books as you would a journal. And when you read a children's book to a loved one, have the child write his or her name and age, write yours (at least your name), write what you feel at that moment, what your young reader said about the book, the date and time and place.
Book signing is the affixing of a signature to the title page or flyleaf of a book by its author. Book signings are events, usually at a bookstore or library, where an author sits and signs books for a period.
Do: Tell them how much their work has meant to you. Don't: Make critiques without an invitation. Do: Thank them for their time. Don't: Ask for a picture with the author if instructed not to by the bookstore staff. Do: Buy their book.
(If I had to give an average, I'd say that my signings are usually about two hours long including the Q&A session at the beginning and the signing line. It, of course, varies a lot event to event and author to author.) DO plan on purchasing something at the store hosting the event if you can.
Step 1: Arrive early. Get there 30 minutes early, at least. You want to have good seats, and they can fill up fast.
Dress professionally In line with making a good impression, wear a professional-looking outfit at your book-signing event. However, this does not necessarily mean that you have to wear a suit or a long dress. You can go for something as casual as a nice jacket with some nice pants.
Decide where to sign. I like to sign my books on the title page, which is where most books are autographed, though you can also sign the inside cover. Personalize your message. Choose a signature phrase. Make sure your name is legible. Add a date (optional). Use a good pen.
