How do I add a radio button to a PDF?
Open the PDF file that you will use for the form.
From the Forms menu, select Add or Edit Fields
From the Add New Field pull-down menu, select Show Tools on Toolbar.
Click Radio Button Tool.
Position the cursor where you want the form field to begin.
How do I select multiple radio buttons?
For windows: Hold down the control (CTRL) button to select multiple options.

Radio buttons allow a user to select a single option among multiple options. You can set the Choice Value of each option, for each button, as well as group these buttons by giving them the same Group Name.
