I have found forms through PDFfiller that I could not locate anywhere else--wonderful tool! It's fairly intuitive, but there are some cumbersome issues related to printing--too many steps--could be simplified.

age. This ROOT-WORD is the Suffix AGE which means ACT OF, STATE OF, FUNCTION, RESULT OF & COLLECTION OF. It is an easy ROOT-WORD to recognize.
Age is just a number, so don't think too much about it. Stay young at heart, and act young, then you will look younger than your actual age (a word here which means the amount or length of time you have spent living your life in this word).
First we'll talk about when you do hyphenate an age: You do it when the age is acting like a noun and when the age is an adjective that comes before the noun and modifies the noun. In this example, the age70-year-oldis used as a noun, and you hyphenate it: That 70-year-old with a purple hoodie loves Justin Bieber.
-ize, suffix. -ize is used to form verbs with the meaning “to make; cause to become:''fossil + -ize fossilize (= to make something into a fossil); sterile + -size sterilize (= to make something sterile).
-ise, 2. a noun suffix, occurring in loanwords from French, indicating quality, condition, or function:franchise; merchandise. Cf. -ice.
age used as a noun: The whole duration of a being, whether animal, vegetable, or other kind; lifetime. That part of the duration of a being or a thing which is between its beginning and any given time.
A lot of people get confused about when to use hyphens when writing ages, and I think it's because sometimes an age is a noun, sometimes an age is an adjective that comes before a noun and modifies it, and sometimes an age is an adjective that comes after a noun.
