Ease of filling the forms. There is one change if you can make will be great... in date of birth column, once you click on it, you have to put the MM/DD/YYYY. If you can allow to override that and just put the year, that will be of help. In my case where my father was born in 1920 and his date of MM/DD is not known, I'm forced to fill it with incorrect information.

Key the information you want to concatenate, and then tap the cell where you want the combined data to appear. Type the equal sign (=), and then type CONCATENATE. Tap the cells you want to combine in the order in which you want them.
Select a blank cell you will place the concatenation result in, and enter the formula =CONCATENATE(TRANSPOSE(A2:A16)&”,”) into it. Highlight the TRANSPOSE(A2:A16)&”,” in the formula, and press the F9 key to replace cell reference with cell contents.
The Microsoft Excel CONCATENATE function allows you to join 2 or more strings together. The CONCATENATE function is a built-in function in Excel that is categorized as a String/Text Function. As a worksheet function, the CONCATENATE function can be entered as part of a formula in a cell of a worksheet.
Copy (CTRL + c) this formula: Double-click on cell F4. Paste (Ctrl + v) formula to cell F4. Select the entire formula in the formula bar. Press Function key F9 and the formula is converted to an array of constants: Delete the equal sign = in the formula bar and then press Enter. Select cell F4.
Add double quotation marks with a space between them “. For example: =CONCATENATE(“Hello”, “, “World!”). Add a space after the Text argument. For example: =CONCATENATE(“Hello “, “World!”). The string “Hello has an extra space added.
Select the cell in which you want the combined data. Type an = (equal sign) to start the formula. Click on the first cell. Type the & operator (shift + 7) Click on the second cell. Press Enter to complete the formula.
