Excellent. The only improvement I can imagine is something to assist with right aligning text, or to keep the same spacing if filling in several columns.

The service produces a very handsome document, and facilitates changes when necessary. I had a little trouble navigating back and forth between the document and the print function, but that may be attributable to my lack of familiarity with the product.

Below is a list of the most common customer questions. If you can’t find an answer to your question, please don’t hesitate to reach out to us.
Most companies use an online, or cloud, storage system with encryption to store your credit card data. Companies are required to store a customer's credit card data using a method that meets the Payment Card Industry's Data Security Standard or PCI DSS.
Your card information is stored in a PCI-compliant external card vault provided by Speedy (www.spreedly.com).
”We're not aware of a law that would prohibit a physician from requiring a credit card on file prior to providing non-emergency treatment.” In short, it seems many doctors' offices want to keep a credit card number on file, so they don't have to wait to get their money.
No, of course it is not illegal to store credit card information. In California, it is against the California Business and Professions Code to record, store or contain sensitive consumer data (yes, Credit Card details are considered sensitive), without adequate security protections, so the answer is — maybe.
Don't wait. Doctors should apologize for delays. And if presented with an invoice for excessive waiting, doctors should gladly pay the fee. Fortunately, most patients don't bill at the doctor's hourly rate.
Credit card numbers should not be kept on file as a general practice. The credit card number must be filed in a secure location, in a safe or under lock and key. Credit card numbers must not be stored electronically, i.e. in a spreadsheet, database, or anywhere on a computer and/or network.
So, your only option is to contact the bank, and prove you are the cardholder, and get a new card issued. For Visa/Mastercard, the three-digit CVV number is printed on the signature panel on the back of the card immediately after the card's account number.
The CVV or CVV2 Number is generated by a specific bank from which you issue your card. The codes are determined by encrypting the card number of the bank and the expiry date with secret encryption keys that are known to the issuing bank only.
The bank can not find the CVV number off the computer, just by interrogating the card's electronic footprint, say through an ATM or Card reader. So each time a new card is issued, a new CVV number is created as well, but endorsed on the back of the card by another machine.
These websites include Amazon.com, Target.com, Armaniexchange.com, NBA.com, eddiebauer.com, bebe.com, Lacoste.com, and benefit.com. Websites like these will require other personal information about you and your card instead of the CVV code, so be sure to have that prepared.
The next time you use your credit card to make a purchase online or over the phone, you may be asked to type in your card's CVV security code first. The simplest way to find your code is to check your card they're typically printed on the front or the back.
For Visa/Mastercard, the three-digit CVV number is printed on the signature panel on the back of the card immediately after the card's account number. For American Express, the four-digit CVV number is printed on the front of the card above the card account number.
Call the credit card company. If you do not have access to your credit card, and you can't find your account number on your statement or online, call your credit card company to get your account number. The number for your credit card company should be located on your bill, or you can look online to find it.
3 — travel/entertainment cards (such as American Express and Diners Club) 4 - Visa. 5 - MasterCard. 6 - Discover Card.
