(a) For paper records containing information that is confidential or exempt from disclosure, appropriate destruction methods include burning in an industrial incineration facility, pulping, pulverizing, shredding, or macerating.
Destruction is the act of disposing of records permanently by obliterating records so that the information in them can no longer be physically or electronically reconstructed or recovered. Destruction decisions must be formally approved before any action is taken.
Patients involved in a clinical trial — 15 years after last episode or 10 years after death (locally agreed exception). Donor records — 11 years post transplantation. Do not destroy — Records marked as a general 'do not destroy' must be kept as permanent full records and should not be destroyed.
A destruction order is a court-issued document requiring that your dog be destroyed. The dog has attacked a person or another animal apart from vermin.
When Medical Records Should Be Destroyed HIPAA requires medical records to be retained for six years from the date of its creation or last use whichever comes later. State laws also generally have document retention laws, however when they're shorter than HIPAA's, the six-year retention period preempts State laws.
Do not destroy — Records marked as a general 'do not destroy' must be kept as permanent full records and should not be destroyed. Be filed in a labelled envelope and stored separate to the main notes. Infertility notes can be destroyed after 8 years providing there are no complications.
Records disposal is the process by which University Records are either destroyed or retained as University Archives. Disposal is a range of processes associated with implementing the records' retention, destruction or transfer decisions documented in the University's Records Retention and Disposal Authority.
Microfilm, microfiche, magnetic computer tapes, compact disks and diskettes must be destroyed in a manner that ensures the information contained in the media cannot be reconstructed. Burning is considered the most effective method of destroying microforms, magnetic tapes and compact disks and diskettes.
