You do not need to be fully fit to go back to work. For example: your employer may agree to make some changes to help you return. If your health condition no longer affects your ability to do your normal duties, you may be able to return, even though you've only partly recovered.
Employers can't require their employees to submit doctors' notes for each FMLA absence. Whether the employer's request is lawful will depend on the specific situation. The general rule is that an employee must provide enough information to put the employer on notice of the need for leave.
There isn't a general legal requirement for your employer to give time off for medical appointments. Some employers may require you to take annual leave, or take the time unpaid. On the other hand, many employers will be flexible on this, and it may be worth asking your employer if you can make up the time later.
Do not feel that you have to disclose every aspect about your health diagnosis to your employer or to co-workers. For example, you may feel comfortable sharing news about a routine knee surgery, but you may keep more intimate or serious medical conditions private.
Employers have the right to ask for a note from the physician when an employee takes time off for sickness, but they must apply the policy equally to all employees. An employer can require employees to turn in a doctor's note when they are off for more than three consecutive days and cite sickness as the reason.
Starting in July 2015, California employers now have to provide their employees with at least three paid sick leave days per year. So it's now the law — if you have employees, they get sick time. The requirement that employees provide a “doctor's note" when they take sick time is nothing new.
If there was one paid sick leave question that dominated, it was about requiring a doctor's note as a condition of either taking a paid sick day or returning from one. Many employers maintained policies that required doctor's notes after three days of unexcused absences.

Does my employee need a fit note saying they are fit for work? No. People do not need to be signed back to work and there is no option on the fit note to do so. If your employee's doctor assesses that they are fit for work, they will not be issued with a fit note.
A doctor's note must always mention the date of the rendering of the note. Antedating is against the law, and can be pursued as a criminal offense. The only thing he can do, in your favor, is noting that the PATIENT declares he was unable to work in the previous days.
It is possible for a sick note to be back dated, so you do not need to be seen urgently or on the day that your sick note expires. It can be issued at a later date and back dated at the doctor's discretion. A doctor may also extend a sick note without you being seen again.
If you need a fit note, contact your GP surgery. Or if you are getting hospital treatment, ask for one from your hospital doctor. Your doctor will assess you, and if they decide your health affects your fitness for work, they can issue a fit note and advise either that: you are “not fit for work"
In some cases, you may not need to have a doctor write you a note, but your organization may require that you submit your own letter explaining why you were absent from work. Check with your HR department for details about what that letter should include and how it should be formatted.
An urgent care center should have a policy in place for handling the issuance of doctor's notes. Though not primary care providers, many centers will indeed issue doctor's notes upon request. Most urgent care providers will believe the patient, and rightfully so, as we're not the illness police.
