The application process for obtaining a U.S. work permit (also called an employment authorization document or EAD) is fairly straightforward. You need to fill out a one-page form, attach the fee, photos, and documents proving you're eligible, and submit it to U.S. Citizenship and Immigration Services (USCIS).
Minor students aged 14 through 17 should take the following steps to obtain and complete their work permit: Request the Department of Education form entitled: Statement of Intent to Employ Minor and Request for Work Permit from their school. You can access the form by clicking here.

If the teenager is not a U.S. citizen, obtain the alien certification number of the parent.
