What do you like best?
I manage customer service at high tech firms but at home I am just a mom. And every summer I have the nightmare of getting medical forms for kids to summer camps. Usually it is a real pain of printing the form, filling in the info for the kid, scanning it, faxing it to the pediatrician. This time I decided to try PDFfiller which I use at work, and it literally took me 3 min to do two camps! I just imported the medical form pdf, filled it in, and faxed it to the doctor's office right from the app. Nightmare no more :-)
What do you dislike?
Some UI feels non-standard. For example I could only save the pdf in my downloads folder rather than being able to select the folder I wanted.
What problems are you solving with the product? What benefits have you realized?
Primarily, filling of paper forms online, for example insurance and tax paperwork for new employees.

A: No. The HIPAA Privacy Rule does not require you to notarize authorization forms or have a witness. Though taking the time to fill out an authorization form and get a patient's signature is an extra step, it's an important one that you can't afford to overlook.
Under the Rule, an individual's personal representative is someone authorized under State or other applicable law to act on behalf of the individual in making health care related decisions.
A HIPAA authorization form is a document in that allows an appointed person or party to share specific health information with another person or group. Your appointed person can be a doctor, a hospital, or a health care provider, as well as certain other entities such as an attorney.
Electronic signatures are allowed under HIPAA and may be used for authorization. The authorization must contain the patient's signature and the date the document was signed.
You Meet HIPAA Standards. For true HIPAA-compliant Healthcare faxing, you can trust fax Corporate®. With eFax Corporate deployed across your organization, your staff can securely fax by email from any Internet connected device including desktops, laptops, tablets or smartphones.
1) Never leave faxes unattended on fax machines. Whenever you need to send a fax that contains PHI or other personal and private information, you must stay right there until you are finished faxing. 4) Keep an audit trail. 5) Migrate to the cloud.
Gmail is not automatically HIPAA-compliant, however, you can implement security measures to ensure the safety of sensitive information you send via Gmail. You need to use a third party email encryption service to protect any PHI you send over Gmail.

Medical Consent Form A minor child can't make decisions about his own health care. It's also good form to state that no court orders prevent you from making such an authorization. You and the other parent should sign the form and get your signatures notarized.
In some cases, a medical release form requires that a patient representative such as a parent or legal guardian sign the form. If such is the case, the medical release form should be accompanied by an additional form that gives such authorization to the patient representative.
HIPAA does not impose any specific time limit on authorizations. For example, an authorization could state that it is good for 30 days, 90 days or even for 2 years. An authorization could also provide that it expires when the client reaches a certain age. In this case, the 90-day expiration date is set by the agency.
