After a slow start and feeling misled by the "fill in your form for free" and then having to pay, I did get a full refund. The tech support was excellent - they were the only ones, that I could find, that could get me the rather complex fill in form from NY State Tax authorities (RP-5217). I still feel I should not have to pay a third party to file a required tax form - but that is an issue I'll have to take up with NYS, wish me good luck with that one.

Systematic record-keeping provides evidence of the degree of care taken by counselors in their work, which may be useful if a client makes a complaint against a counselor to a professional body or begins legal action against a counselor. It also protects against differences in memory between client and counselor.
Retention of records The guidelines state: “In the absence of a superseding requirement, psychologists may consider retaining full records until seven years after the last date of service delivery for adults or until three years after a minor reaches the age of majority, whichever is later.”

A counseling note is also referred to as a psychotherapy note, a process note or a private note. It contains the hypotheses, observations, thoughts and further questions the treating mental health professional may have about the patient during a counseling session.
Step 1: Know your brief. You will usually receive a clear brief for a report, including what you are studying and for whom the report should be prepared. Step 2: Keep your brief in mind at all times. Executive Summary. Introduction. Report Main Body. Conclusions and Recommendations.
Essentially, a report is a short, sharp, concise document which is written for a particular purpose and audience. It generally sets outs and analyses a situation or problem, often making recommendations for future action. It is a factual paper, and needs to be clear and well-structured.

How Long Does the School Need to Maintain IEP Documents? The type of student record and your state's laws will dictate document retention requirements. Some special education records must be kept for at least three years after the student leaves the district.
IEPs do not expire. An IEP remains in effect until a new one is written, or you agree that an IEP for specialized instruction and related services is no longer needed. Removal from special education requires prior written notice from the school.
A student's right to privacy and confidentiality is the basis for an effective counseling relationship. Confidentiality ensures that school counselors won't share students' disclosures with others except when the student authorizes it or when there is a clear and present danger to the student and/or to other persons.
A: The short answer is yes, but it's complicated. School counselors have the challenge of balancing their students' rights to confidentiality with the legitimate rights and concerns of teachers.
Under Education Code section 49602(c), is a school counselor required to disclose pregnancy-related or abortion-related personal information received from an emancipated student age 12 or older to the student's parents or school principal when the counselor has reasonable cause to believe that disclosure is necessary
Thus, school counselors are mandated reporters. As mandated reporters, they and other school personnel are required by law to report suspected child abuse and neglect. CPS should be notified as soon as abuse or neglect is suspected.
In some cases, distressed students may even disrupt the learning of others. A referral to Counseling Services should be considered when you believe a student's difficulties go beyond your own experience and expertise or when you feel uncomfortable helping a student with an issue.
