Very good so far except that when I attempt to print completed documents the program changes the orientation of the doc so that it cannot be printed. I've managed a work around, but it's annoying.

It's never been a common practice in the U.S. to give two middle names. But with families having fewer children in general, the temptation to bestow two middle names to honor several parents, for example is strong.
If you have two middle names or if you're giving to someone with two middle names, it's typical to just use one middle name for the monogram. Four letters are often too large, and it's really awkward if you want to put your last name in the middle. It just looks off balance.
If all the letters are the same size (also known as block), initials are ordered like your name: first, middle and last. If the monogram features a larger center initial, the ordering is always first name, last name, and middle name.
