It's an awesome program. The only problem is that this form is used with all insurance companies and the top should be blank for that particular company to fill in their name.

Great. Had trouble logging in as I kept getting redirected to "PDFESCAPE". I'm not a computer person, but couldn't get here until I actually typed in your complete e-mail address. In other words, if I type in "PD" in my browser and "PDFFILLER" automatically appeared and I clikced on it, I was redirected to another website.

Below is a list of the most common customer questions. If you can’t find an answer to your question, please don’t hesitate to reach out to us.
Facial Reconstruction. Facial reconstruction is a method used in the forensic field when a crime involves unidentified remains. Either way, the sculptor will work with forensic anthropologists to interpret the features of the skeleton that will ultimately help reveal the age, sex and ancestry of the victim.
In manual methods, facial reconstruction is done by using clay, plastic or wax directly on the victim's skull or more often a replica of the skull which has to be identified.
The skull is the basis of facial reconstruction. However, other physical remains that are sometimes available often prove to be valuable. Occasionally, remnants of soft tissue are found on a set of remains. This eliminates one of the most difficult aspects of reconstruction, the estimation of tissue thickness.
Head and neck surgeons also perform craniofacial reconstruction operations. The surgery is done while you are deep asleep and pain-free (under general anesthesia). The surgery may take 4 to 12 hours or more. Some bones of the face are cut and moved.
Facial reconstruction is accurate because it has been using mathematics, and calculations as such, in order to determine spaces, amounts, as well as the shapes of the face themselves. Facial recognition, as well as facial reconstruction is very important in the forensic field.
Characteristics such as birthmarks, wrinkles, scars, and such are guesses at best and cannot actually be determined from the skull. Facial reconstruction is accurate because it has been using mathematics, and calculations as such, in order to determine spaces, amounts, as well as the shapes of the face themselves.
The 3D manual methods used in forensic facial reconstruction are the Anatomical (Russian), Anthropometric (American) and Combination Manchester (British) methods which were developed by Asimov, Frogman and Leave respectively [2,8,9].
There are limitations to the facial reconstruction by the artist/sculptor. The hair color, hairstyles, facial expression, shape of lips, and eye color, and how much fat on soft tissue areas are generalized guesses.
