At first, I didn't understand what I needed to do first. I filled out the form I pulled up and then submitted it, but was sent to a page to choose which plan I wanted. My list only lhad a previous form from 7/15/2015. I chatted with a representative and she helped me get my second version on my list. prepare your base document, and save. then save as onto your computer desktop, and rename once downloaded onto your computer desktop. reuse the base document over and over for different clients.... works great and gave me the exact form i was looking for.
