I am a self represented defendant in a small claims case. I cannot afford an attorney. Always put a contract amendment in writing and make sure both parties sign and date it. Reference the title of the contract, if applicable. Its original parties. And original signing date, so that it is clear what document you are amending. Attach the amendment to the original contract.
Write, Agreement to Amend Contract at the top of the pertinent page. Enter the names and titles of parties involved. Clearly state in a sentence or two that both parties are agreeing to amend this contract on such-and-such date and such-and-such time. Then clearly describe the changes in writing.
