Awesome! If there is a way to add your logo I could not find it... then again I didn't look either so...this might not be accurate. So easy to use. extremely user-friendly. The benefits are that you get to create any type of form you need and customize it to fit your needs. You cannot ask for anything better than that.

When there are two items in a list, don't use a comma to separate the list items (unless it helps your reader). If there are more than two list items, then those following US convention should use a comma before the conjunction (usually and or). Those following UK convention should not use a comma.

Usually, we use a comma to separate three items or more in a list. However, if one or more of these items contain commas, then you should use a semicolon, instead of a comma, to separate the items and avoid potential confusion.
Rule 1. Use commas to separate words and word groups in a simple series of three or more items. Example: My estate goes to my husband, son, daughter-in-law, and nephew. Note: When the last comma in a series comes before and or (after daughter-in-law in the above example), it is known as the Oxford comma.
When writing numbers, make sure you use a comma to separate numbers over 1,000. This rule helps to make numbers easier to read. For example, The road is 458, 987 miles long, rather than, The road is 458987 miles long. The same applies to tens of thousands, millions and billions, etc.
A simple rule for using numbers in writing is that small numbers ranging from one to ten (or one to nine, depending on the style guide) should generally be spelled out. Larger numbers (i.e., above ten) are written as numerals.
Rule 1. Spell out all numbers beginning a sentence. Rule 2a. Hyphenate all compound numbers from twenty-one through ninety-nine. Rule 2b. Hyphenate all written-out fractions. Rule 3a. With figures of four or more digits, use commas. Rule 3b. Rule 3c. Rule 4a. Rule 4b.
