It seems it is a bait and switch deal. You think you are getting a month for free but to really use the form without watermarks like SAMPLE across it, it seems you have to pay.

PDF Filler has been a great tool for our business for a professional look with a great price. Professional Look for a cheap price. The software makes filling out government documents / applications and other PDF documents easy and efficient. PDF Filler provides and saves time when filling out applications / tax papers / and other business files. It creates a professional look and is quite easy to use. With common files also at hand it makes filling these out quick and easy. The mail tool was even used when we completed our taxes. PDF Filler printed the paper out and mailed it themselves without us ever having to go to the post office. There isn't too much. If I could say anything sometimes the website moves a little bit slower than some. However this seems to have been improving lately.

Accessing past tax records, W2 or 1099 forms, or pay stubs. Submitting a Request for Social Security Earnings Information Form (requires fee) with the Social Security Administration. Contacting previous employers' human resources departments.
The best way to get the exact date of your employment is to call the human resources department of your previous employer, if the company is still in business. If not, call the Internal Revenue Service or the Social Security Administration. They keep records of your work history.
Can employers access your employment history via social security? No. Your social security number can be used to verify your identity, but it can't reveal your work history. For that, the employer would need you to provide a list of the places where you've worked.
Obtaining Employment History from the Internal Revenue Service. Visit the Internal Revenue Service's website at www.IRS.gov. Click Forms and Publications on the left-hand navigation sidebar. Search for IRS Form 4506.
A free option may be to get records from your state's unemployment office. If you've moved around a lot, this could prove difficult, but if most of your employment was in one or two states, you can request these records and reconstruct your job history. Check with your state to find out what is available.
