Include details or insight that wouldn't necessarily show up on a resume or application. Your letter should help the recipient get to know the person you are recommending.
You can't do anything. In principle, the professor can resend the letter, either by revisiting the portal or by emailing the admissions committee directly, but it's really not worth the trouble.
The most important thing about a letter of recommendation is its credibility. So it's better not to make any edits on your own. In case they are absolutely necessary communicate any corrections with the person who issued a letter.
