The MV-1 Form is the State of Georgia Department of Revenue (FOR) form that must be completed by the vehicle owner for most vehicle title & registration transactions, including original applications, renewals, replacements, and changes.
What is the purpose of the MV-1 Form?
The MV-1 Form is the FOR Motor Vehicle Title/Tag Application that can be completed for several primary purposes:
As an initial application for a new vehicle title and/or a license plate regardless of the conditions of its purchase;
As an application to recover the vehicle title and/or tag due its loss or damage.
All of these actions should concern the vehicles registered in the State of Georgia.
Is the MV-1 Form accompanied by any other forms?

How to fill out the MV-1 Form?
When filled out, the Georgia Vehicle Title/Tag Application must provide the following information:
Data about the vehicle including ID number, year, make, technical details, date of purchase, current tile, etc.
Details of the owner (or owners) including number of the driver’s license, full name, date of birth and the origin of the car;
Information about the seller and lessee if applicable;
Terms of security interests or lien.
Finally, the MV-1 Form must be signed by the owner or owners.
