The issue was not being able to recreate the form in that most environments you would need to build the form from scratch. PDFfiller allowed me to upload the form and use their platform to publish it to the public. May I add how quick and easy it was. Saved a ton of time which I don't have.

How do I attach the customs documents to the box? The best thing to do is use a 'documents enclosed' pouch which is designed to be attached to a box for shipping packages. A clear plastic wallet or envelope is also suitable as long as it is taped down so the customs documents don't move in transit.
In the Shipment History window, select the shipment group for which you want to reprint documents. On the Printing Activities tab, select International and then the document you want to reprint. Tip: If a document is not available for the selected shipment, that selection is either inactive or does not appear.
Simply include a copy of the form inside the package, then attach the short form or label to the outside package. Make sure you use transparent tape (or put it in the plastic envelope attached to the package) so that the form can be clearly read. Don't forget to keep your sender's copy with you for future reference.
When your shipment is displayed, select the shipment you wish to reprint the receipt for. Select the Show Detail/Receipt button. Select the View/Print Receipt button.
